How to Cross-Sell Life, LTC, and Annuities from Medicare Appointments

Most agents treat Medicare as a one-and-done sale. But what if your Medicare review could open the door to three additional policies per household—without being pushy or salesy?

The truth is, your Medicare clients have three major gaps:
• They’re unsure how to create predictable income in retirement
• They’ve never made a plan for long-term care expenses
• They haven’t protected their estate from taxes or final expenses

At Agency Engineer, we teach agents how to bridge these gaps during a Medicare appointment using our Medicare Cross-Selling Playbook. Here’s how to do it step-by-step.

Step 1: Set the Tone Early — You’re More Than a Medicare Agent

If you start by saying, “I’m just here to help with Medicare,” you’ll limit your role from the jump.

Instead, open with something like:

“Today we’ll go through your Medicare plan options and make sure your healthcare is squared away. But I also help clients plan for income, caregiving, and leaving a legacy—so if those come up in conversation, I’ll flag a few things for us to revisit.”

Now you’ve set a dual-expert expectation without pitching anything.

Step 2: Listen for Triggers as You Review Medicare

Most of your opportunity comes from what they tell you indirectly.

As you walk through their Medicare 101 options (use the [Medicare 101 Guide]), listen for:
• Income anxiety: “I’m on a fixed income now…” or “We’re waiting to take Social Security.”
• Long-term care fear: “My mom needed a facility and it drained her savings.”
• Estate concerns: “I just want to leave something behind for the kids.”

You don’t need to solve anything on the spot. Just flag the concern and tee up a second meeting:

“That’s actually something we help clients plan for all the time. Let’s finish Medicare today, and I’ll set a time for us to explore that in more depth.”

This creates follow-up meetings with purpose.

Step 3: Ask One Power Question Per Topic

Here are 3 questions that get clients thinking differently:

💡 Life Insurance

“Did you know the Social Security death benefit is only $255? If something happened to you, would your family have what they need to stay secure?”

💡 Annuities / Retirement Income

“Do you know how you’re going to turn your savings into income once you stop working?”

💡 Long-Term Care

“If you needed 3 years of care at $100,000 per year, how would you want to pay for that?”

These aren’t sales pitches. They’re conversation starters that lead to a solution-focused second meeting.

Step 4: Book the Next Meeting Before You Leave

Don’t just say, “I can help with that later.” Make it concrete.

Here’s a great line:

“Let’s do this: I’ll send over a Retirement Essentials checklist to help you prep. Then we’ll hop on a second call next week to walk through solutions. What day works best for you?”

This works because:
• It gives the client time to prepare
• It frames you as their retirement guide, not just an insurance seller
• It builds continuity in the relationship

Step 5: Know What to Offer in Meeting #2

When you book the second appointment, show up ready. Use the [Medicare Cross-Selling Playbook] to:

LIFE:
• Review any existing coverage
• Offer final expense, term, or permanent depending on goals
• Position it as a way to protect their legacy, not just pay for burial

LTC:
• Explain what Medicare doesn’t cover (beyond 100 days)
• Discuss home care vs. facility preferences
• Introduce options: traditional LTC, hybrid life/LTC, annuity with LTC rider

ANNUITY:
• Use the “bridge to Social Security” narrative: defer benefits, but create income with an annuity
• Offer guaranteed income for those afraid of outliving their money
• Position FIAs as growth alternatives for clients wary of stock market volatility

Pro Tip: Let the client guide the order of interest. You don’t have to cover everything in one sitting. Build trust. Offer to circle back.

Bonus: Use Tools That Help You Look Like a Pro

At Agency Engineer, we give you:
• White-labeled [Medicare 101 presentation] to teach instead of sell
• Use the [Legacy Essentials] and [Retirement Essentials] presentations to supplement your Medicare 101 visuals
• Guide the conversation with prebuilt scripts and second-meeting templates

These tools help you show up with confidence and avoid sounding like every other agent.

The presentations mentioned above are available for free when you contract with Agency Engineer or purchase the full resources package.

Real Agent Example: Medicare → LTC → $80K Annual Premium

One of our top agents helped a couple choose a Medicare Supplement Plan G. During that conversation, they mentioned caring for an aging parent.

He asked the LTC question. That led to:
• A second appointment
• A review of care preferences and budget
• A hybrid life/LTC policy funded with IRA dollars

Result? $80,000 annual premium and a plan the client was thrilled about.

All from listening carefully during a Medicare conversation.
